Aloha Flight 243

On April 28, 1988, Aloha Airlines Flight 243 took off from Hilo, Hawaii, bound for Honolulu International Airport, Honolulu, Hawaii. As it reached its flight altitude, the cockpit crew heard a loud noise: behind them, a section of the roof of the passenger section had just blown off the aircraft. The crew was able to land the plane, but a flight attendant was sucked out. Her body was never found.

The roof blew off due to an explosive decompression due to stress cracks in the roof of the Boeing 737. A huge investigation was launched subsequent to the accident to determine how something like this could happen, and how it could be prevented.
